Understanding Mastectomy

A mastectomy is a surgical procedure to remove one or both breasts, partially or entirely, as a treatment for or prevention of breast cancer. This procedure can be a life-saving treatment for many, but it also poses significant emotional and physical challenges. That’s where breast reconstruction comes into play, offering women a path to reclaim their body’s appearance and feel more like themselves again.

Types of Mastectomy Procedures

  • Simple or Total Mastectomy: Removal of the entire breast without lymph node removal.
  • Double Mastectomy: Removal of both breasts, often chosen as a preventive measure.
  • Radical Mastectomy: Removal of the entire breast, chest wall muscles under the breast, and all of the lymph nodes under the arm. This is less common and usually reserved for extensive cancer cases.
  • Partial Mastectomy or Lumpectomy: Removal of part of the breast tissue, often followed by radiation.

Breast Reconstruction Overview

Breast reconstruction is a surgical procedure to rebuild the shape and look of a breast, most commonly following mastectomy. Deciding on reconstruction is a highly personal choice and can be performed at the same time as the mastectomy (immediate reconstruction) or at a later date (delayed reconstruction).

Types of Breast Reconstruction

Implant-Based Reconstruction

  • Direct-to-Implant: This method involves placing a breast implant directly after mastectomy, avoiding the need for tissue expansion.
  • Two-Stage Reconstruction: Involves initially placing a tissue expander to gently stretch the skin and create room for a breast implant, followed by the placement of the implant at a later surgery.

Autologous or Flap Reconstruction

  • DIEP Flap: Uses fat, skin, and blood vessels from the lower abdomen to reconstruct the breast.
  • TRAM Flap: Transfers muscle, fat, and skin from the abdomen to the chest to create a new breast mound.
  • Latissimus Dorsi Flap: Utilizes muscle, fat, and skin from the upper back when abdominal tissue is insufficient.

Nipple and Areola Reconstruction

This can be performed as a separate procedure after the initial reconstruction has healed, using tattooing or skin grafts to recreate the nipple and areola.
